2005 GMC Sierra 1500 coolant leak

Hello, I'm new here, but I recently found a coolant leak. I drive a 2005 GMC Sierra 1500 5.3L 2WD. The problem is there is wear on one of the coolant hoses causing a terrible leak. This hose is connected to the coolant reservoir. This hose goes to the radiator and somewhere towards the cab. Taking out this hose would make a Y shape. Hope this is enough information. I do have pictures, but I dont think I can do that. Thanks in advance.

You will likely need to post a pic, unless someone here happens to know what you are talking about. Where on this hose is it leaking? And is there some form of connector on this hose that splits the hose so it can go to both the rad and the cab of the truck(which should make that hose a heater hose)? If so, fix the spot where the hose rubbed through to prevent it rubbing through the new hose.

Here is a pic of the damage. Finally found out how to upload pictures. Just wanna make sure the image replied is correct with the actual hose. It is the hose connected to where it says ENG UP. You can see the damage. Thanks again guys.
