99 Suburban rough idle/hesitation

I have a 1999 Suburban 2500 4x4. It has a little over 220k on it. I dont know much history but the rig is immaculate inside and out. 

I have noticed that while stopped for a light or stop sign that the idle will fall almost to the point of stalling. Once I remove my foot from the brake it goes away. This is the only time I notice it. I am hearing a possible vacuum leak under hard acceleration but only briefly. Not sure if the two are related. Any and all help is appreciated.

For sure check for a vacuum leak. I prefer to smoke test for that instead of spending a lot of time with brake cleaner or carb cleaner.

 

Possible that the brake booster is partly to blame for the near stalling but I'm only guessing without having the truck in front of me.
