05 2500hd steering wheel alignment

It appears when my wheels are straight my steering wheel is turned 45 degrees when it should be centred.

 

Tried removing steering wheel and repositioning. But it only goes on one way.

 

Tried setting wheel at center and adjusting tie rods. That doesn't fix the problem because if I adjust to correct steering wheel position there won't be enough threads. So that won't work. 

 

Steering box is a 3 groove pitman arm type. So it only goes on one way. BTW, anyone know the difference between 3 and 4 groove steering boxes, besides the grooves??

 

Steering shaft to steering box coupling has one groove, so it only fits one way.

 

Been like this since I owned the damn thing. Drives fine otherwise. It bothers me, I'm bored and I want to fix it.

 

Am I missing something here guys?

sounds like wrong pitman arm. This can happen if a used box was put in, and the hollander manual said it fits, but the yard grabbed it with the pitman arm, and it was from a different vehicle.
