How does the computer know how much fuel is being used? Accuracy? Does it base its flow rate using the pulse rate of the injectors, knowing the fuel pump pressure? Or just a simple flow meter?

The fuel used is based upon fuel pressure/injector flow rate/injector pulse length. Just like you were thinking it knows how much fuel passes through and injector during a period time at a given pressure.
