1995 Yukon Front Brake Issue

I have a 1995 GMC Yukon. It is the 5.7 liter motor and is the 4WD model. I have gone through 3 rotors now on the right front in the past couple of years. For some reason, the inside brake lining on the passenger side brake pad keeps delaminating from the base plate and I end up metal to metal destroying the rotor.

The first time it happened, I thought defective pad. The second time, just last year, (note, the same pad location failed) I asked the parts guy what he thought. He said the brake lines might be doing the strawberry shake thing where they are collapsing and not letting the pads fully release thus dragging and causing extra heat on that pad. Either that or there is an issue in the caliper itself.

So, I changed out the rubber lines, calipers, rotors, and pads. Now here I am about a year later and I lost the right inside pad and rotor again. I just don't know where to look now.
