Chevy silverado clunk noise when ur barely moving

my truck has 270000 miles on St but it runs I great except when u barely move the truck especially on grass or ur small hills it sounds like the rear end clunks and if u punch the gas and spin a Lil bit it slips in the rear of the rear of the shaft I've replaced all joints and greased yoke what is this problem?

With 270,000 would a little wear in the ring and pinion perhaps allow it to clunk? Try your Chev dealership for a diagnostic test.
