1990 Cheyenne K1500 Fan/Engine whine? (Constant)

The whine gets faster and higher as it revs. It is constant, while idle, everything. Its always done it and were trying to figure it out. Heres a video, any ideas on what it might be? Think its the fan.

Literally its been doing this for almost 5 years, but yeah could be. The PS has got really high pitched lately.
