serpentine belt jumping and damaging pulley

Has anyone else with a 6.0 had the serpentine jump and damage one of the lovely plastic pulleys in the process?

I had one come off in a Colorado it popped off most likely from a stone. This happens more often then people think.

 

My mother had a belt tensioner fail in an Escort that did a pile of damage.

 

I actually just change the Tensioners at a 100K now. It's cheap insurance.

 

Make sure they check the tensioner....
