06 avalanche overcharging

hi i have a 06 avalanche and i noticed that most the time it charges at 16v and some time 12v never where it should thought i know with this year its supposed to fluctuate but shouldn't be charging at 16v most the time. my phone charger will shut off because the voltage go's to high.if you have any ideas please let me know.thank you
