I am getting ready to do just what you are asking about in my 2005 Silverado. What I would suggest is either go to gmpartsdirect to see if they have the part or a dealership. I was able to get the complete kit through my dealership for $225. Gmpartsdirect had it cheaper but then the shipping put it over the dealership cost. Here is a link to another forum which told me about doing the install and kit; http://www.fullsizechevy.com/forums/showthread.php?t=149544 The part number I ordered is 12499673, can't say for sure if it will work on your particular year. But the kit comes with switch, wires, new valance, lights.

Hope this helps and have a great day.

I have same year truck as Busta and had to have the computer reprogrammed to get the add-on factory fog lights to work. You might consider using factory lights and just connect to swtiched 12V with relay.
