To Much Fuel

I've got a 93 half-ton 4X4 with a 350, When i got the truck it always smelt rich, hooked up a scanner and ECM is telling injectors to spray way to much, as far as i know it's the Oem ECM and hasn't been played with. i swapped engines and added some headers and a cam, And i still have the same problem. Any ideas besides getting ECM reprogrammed?

Bad MAP sensor maybe. Does it do it in open loop, closed loop, or both. Could the O2 at that point if its doing it in closed loop.
