If you haven't changed the fuel filter I would say that would be a worth while expense. Once done see how it does and go from there. That's my thoughts.

Try cleaning the MAF? Air Filter? Double check plugs?

Maybe even a possibly bad fuel pressure regulator? (Not sure how you would test for that though)

i have an aftermarket k&n air intake system filter brand new i could check the maf sensor someone had mentioned plugged cats or bad 02 sensor that possible????

Yup, they certainly aren't going to help if plugged up or if they are shot. With as many miles as you have I'd be looking at everything from intake to exhaust just for routine maintenance. Hopefully along the way you'll find the problem.

what would happend if i took the cats out would the truck run like crap cause when the truck running its a hundsred percent and sometimes stalls piking up speed

Possibly, I'm not a certified GM mechanic, equipment I work on doesn't have emissions. Everything I've learned about these trucks has been from this forum. Take a look in the Engine & Powertrain section for some ideas on what may or may not work.

Sorry I couldn't be more help.

Have you checked the fuel pressure regulator? Have you checked actual fuel pressure reading?
