2010 2500hd cabin fan works intermittently

my cabin fan seem to be possessed, it works when it wants to. no matter what the fan or heat control settings it will come on right after start up or some time while driving or not at all. once working everything works perfect. when fan is not working it appears the other controls are ( I can here the heating actuator moving) any ideas ?

There is a blower motor module that runs the speeds for the fan, perhaps that is failing.
