Bad Throwout Bearing?

In 3rd and 4th gear, shifting into them with medium throttle makes a gravely, slight grating noise. Also when I shift into reverse, immediately after truck has come to a halt, gears grind for a second and then reverse goes in smoothly. What is up? New trans fluid about 1000 mi. ago, and no leaks from the transmission. Bad throwout bearing? Worn out synchros? Or a transfer case issue? I know my transfer case leaks slightly and I havent checked its fluid levels, but the shift to 4Hi and 4Lo is smooth.

The clutch might not be engaging all the way or bad transmission but for the reverse try slowing the engine rpm down befor shifting by putting it in gear and letting the clutch out slightly to slow the rpm down befor shifting but no big deal mine does it all the time. If it were a throw out bearing you would hear a noise when ever the clutch was pushed in and the engine was running
