pingin/knockin bad

I have a rattling in gear when I accelerate. If i put my truck in neutral it doesn't do it. It sounds like a rattling/chattering kinda noise not sure what it is or how to describe the noise. I know its there. I like many others have the CSK but it goes away. Its coming from the engine compartment. I have tried cleaning the MAFS and throttle body. I have used techron fuel system treatment and I am at a loss. Anyone else experience this?
