When I turn my control to 5 my fan stops?

I hear a click under the passenger side dash at the same time?

Well I lived with this issue now for almost a year.

Now 1 & 2 stopped working.

This relay switch, is it connected to the fan motor on the passenger side?

Is it the relay under the dash on the driver side?

Is is the switch in the dash itself?

Yes, if you follow the two wires from the blower motor back you will find the blower resistor. It has about 5 wires plugged into it from the body harness. This is the part you would need to replace. The blower motor resistor.
