Loud noise at start up

Here's the issue, just in the last few days, when I start my rig (03 2500HD 6.0L) it sounds as if I have it wide open for about 1-2 seconds. Only can be duplicated when cold and not every time. Because of this I am holding off taking it in. Engine only has about 6K on it. Thoughts?

What is the tach reading? A quick burst of 1500 RPM or so is probably normal.

Well friggin' duh! You would think I'd look at that but alas I haven't. Man I'm stupid sometimes. I'm really not explaining it right. It is more like a gunshot. Very loud and very fast. And the puff of smoke. I called three dealers today and they all were puzzled by this one. I can take it in but of course if it can't be replicated then there's nothing wrong. Hopefully it threw a code. I plan on taking it in on Thursday as I fly to LA tomorrow.
