rough idle and then clears up

Hi my 95 chevy 3500hd has a rough idle when you first start the truck. When you try and rev it up, its like nothing is happening, might go up 2-300 rpms....nothing after that. After about 10 seconds it clears up and off I go like any other day. Anyone know anything about this

maybe fuel pressure? is there a regulator or maybe a weak pump? My new problem as of today is that my top end mph is suffering, im having a hard time moving the truck 2300rpms in 5th gear, thats topped out...I use to be able to go to 3300 no problem (rarely did that tho)....I have a power loss....its almost like after a 1/4 pedal, the rest of the gas pedal is the same, I can floor it and the rpms just go up slowly in each gear....anyone?

Well I decided to replace the fuel cap,lift pump and the ops and im still having the rough idle for 10-20 seconds then clears up, but ive still lost some of my top end.......

almost sounds like a bad glow plug or low compression.
