All might not be lost.

 

IF the backing board is coming loose from its mounts, you can urethane the plastic (like plastic velcro) mounting points back to the roof structure quite sucessfully...but if the material is coming off of the backing board, it's time for a removal of the assembly and replacement of the cloth. Not too bad on a regular cab... kind of a PITA on excab or crewcab models unless you pull the back glass to allow the assembly to come out without bending it.

The headliner in my 06 crewcab was sagging when I drove it off the lot. It sags dead center in the middle where the light group is. Its been to the dealership twice in an attempt to fix it. Last time it lasted about a week and then dropped again. I havent taken it back since. They keep my ride for at least a day to fix it without success. The last attempt was double sided industrial velcro on each surface.

 

I've checked it out on all my friends who have 05+ crew cabs and it sags on all of them.
