Leaf Spring out of alignment???

I just bought a 2008 Sierra crew cab, 5.3. It has a small body lift on it with 285/70/r17 M/T tires. I had the truck for about a week before I noticed it making a loud noise when hitting bumps. I did some investigating and found it to be the driver side leaf spring was out of alignment. One of the leafs was so far out it was rubbing on the shackle. What I want to know is what could be causing this to happen?!?

Is it just as simple as worn out bushings or totally worn out leaf springs? The truck has 95000 on it if that helps. I'm attaching a few pictures so you can see what I mean.

Any help is greatly appreciated!!!
