Transmission Noises

I have a 1995 k2500 7.4 ltr i was driving at highway speeds when i felt and heard a loud clunk. then my gears started to grind. I tried all gears in 2 hi and 4 hi and all I got was a loud grinding noise. When I put the truck in 4 lo I was able to move and no longer heard the noises. Is this a transmission problem or a transfer case problem. I also would like to know of any temporary fixes i might be able to do. I am 600 miles from my tools and spare parts. I also forgot to mention that it is a manual 5 speed transmission.
