Gas Gauges

ok my truck starts great, idles great but everytime i stop at a light or sign and try to take off it chokes out badly. Has yet to die. Gas gauge is irractic at best and this only happens when it appears to be empty. i feel there must be something clogging the gas lines or the float is cork maybe and fallen apart in the tank? i've had a complete tune-up and new fuel filter and even been using injection cleaner for the last few tanks of gas but still keeps happening when the gas is low. it has never happened when there is more than a 1/4 a tank of fuel. Could this be possible? or could there just be a foreign object in the tank? what can i do to fix this before it dies in a dangerous place.

Your fuel sending unit is certainly in need of replacing. (Fuel Pump AND the fuel level module) That may or may not cure your rough performance issue though.
