Coolant leaking from "temperature actuator"?

I have a '98 GMC Savana 1500 conversion van with aftermarket rear heat/AC that was part of the original conversion. 5.7L V8. 142k miles.

 

The van has a coolant leak that's coming from a part used to control coolant flow to the heater cores under the hood, behind the AC accumulator. I have the Haynes repair manual and it incidentally shows some photos of the part, but no direct mention of it anywhere except an electrical diagram of the heating/air conditioning system, where it's labelled a temperature actuator. I've included pics of the leaking part below.

 

Some googling of parts did not turn up much. What is this thing called? What part do I need?

I'd call it a heater valve. I believe that's how several parts locations would call it.

 

 

AC Delco # 1550089

 

Four Seasons # 74781

 

 

On rockauto.com it shows theAC Delco comes with 2 of the hoses that have 90* bends in them. That's the one I'd buy
