What exactly does this thing do?

It goes in place of where the license plate is mounted on the front of the truck. In Indiana we don't have to have front plates, unless you put a vanity plate on there. If you like front plates then no issue you don't need it, otherwise it give the front of the sierra a very clean look instead of gaping hole.

OK cool thanks for the info! We are required to have front plates in Ohio so I guess that's why I've not heard of one of these things before! GLWS
