One of your O2 sensors is probably going bad. 

 

If not that then somehow you have messed up your trucks tune causing the AFR to be funky

When a code P219A is stored, it means that the power train control module (PCM) has detected an air to fuel ratio imbalance for the bank of the engine which contains the number one cylinder. In order to monitor the air to fuel ratio for each engine bank, the PCM uses data input from heated exhaust oxygen sensors (sometimes called air fuel sensors).

I have a broken exhaust bolt on that side (one on the other side too) and wondering if this is the cause. I also have the AFM/DOD tuned out of the ECM but nothing else was messed with…
