I just purchased a 04 gmc safari 4.3L, 37000 miles. runs good but has a rough idle worse when cold. It was a GM certified vehicle ( IF that means much?) Im big on maintenance. It was not serviced very well from the dealer. The oil had not been changed in some time. I changed the oil, filter, fuel filter, pcv valve when I got it. I plan on changing the plugs and cleaning the EGR valve and possibly the throttle body. Would there be anything else that would cause the rough idle? Thanks

Lots of things can cause a rough idle.  There's nothing common on these.  Do you want a wild guess?

Well, I dont want a wild guess. It also gets pretty poor gas mileage (12PMG). Should I have to change plugs by now? Is cleaning the EGR and TB a good idea?

It's pretty early for needing a plug change. Cleaning the TB wouldn't hurt, though I don't know if it'd help anything. I'd say attempting to clean the EGR would be a waste of time and effort.

I'd look in the distributor cap for corrosion, but that would cause more than a rough idle. Injectors may need cleaning, but again, not a common issue on these.
