Damn that sucks. Definitely didnt put the lift pads on the right spots or left em hanging off. I was always taught to lift the car so the tires are an inch or 2 off the ground and shake the vehicle quickly to make sure it is sturdy on the lift.

looks to me like the owner got something out of the truck and slamming the door after caused it to fall.. obviously that lift was not designed for a vehicle of that size.
