Back flip-seat squeak and rattle problem

I have a 2009 Silverado crew cab Z71 4x4 with the A68 rear folding 60/40 bench seat.

 

A while back while transporting back seat passengers (light women, by the way), there was a loud crunch/clang noise and they said it felt like the seat broke. We stopped and they got out and we all looked, but the nuts are tight on the floor bolts. The seat top has play it in now, and there are some black plastic pieces (from broken brackets?) laying on the floor under the seat.

 

Now it rattles and squeaks like crazy, especially with the seat in the down position.

 

Can anybody give me the part numbers and quantities for those broken black plastic brackets behind the rear seat? Apparently they act as a sort of bushing between the top part of the back seat and the rear wall of the cab. I think maybe there are 3 of them, 2 on the '60' side and 1 on the '40' side of the seat, but am not sure and cannot find them on parts sites.

 

If those parts are that weak, I figured this was a common issue, and I'd like to order the parts on the 'net to have them ready to install when I take the back seat out.

 

Also, if anyone knows of special tools needed or tricks to know to take out these rear split seats, please let me know.

There are 3 of them on the back of the back seats (2 on the 60% and one on the 40% side. Part #s I have no idea, but all of mine broke a year or so back. I'm just going to pull the seat out and wrap the hook part with heat shrink tubing or something instead of replacing them and having them break again.
