Do I need a new wiper blade transmission?

Passenger side blade arm works with no issues.  The driver side needs a nudge to move. When  nudged, the arc of the wiper takes the blade right off the windshield.

Sounds like a broken gear tooth

Turns out the nut connecting the wiper arm to the stud, was loose. Tightened it up and I am all set.  I heard this happens over time, especially if you run the wipers if they are stuck in frozen ice or snow
