Does anyone recognize these wheels? A semi hit my truck, tearing up both the tire and wheel. They were 18 inches. How do I describe it when I'm looking for them? Better yet, is there a model number or something than can be used to identify them?

It's the other guy's insurance company's problem to locate a new wheel for you, not yours. If they offer you money (they will) I'd take the money if it's at least the cost of a NEW wheel.  Then I'd go out and buy a good used wheel and pocket the difference. They will of course want to give you the cost of a used wheel because the wheel that was damaged was used... and they will want to pay you for a used tire.  You'll counter that with the fact you can't get a road hazard warranty on a used tire. You want the cost of a new tire, wheel, mounting, balancing, and the road hazard warranty. What I usually do is offer to find and install the parts for the costs (locating, shipping, etc.) of the part and a couple hundred or more in labor; if you can save the adjuster some $ you can make some money....they are happy (not your priority) and you (your priority) are happier.

Inside your glove compartment (I think) is a "Service Parts Codes" label with a bunch of three digit codes.  One of those will tell you what kind of wheels your truck has.  Then search on that.  I'm not sure which code it is though.

Check with places that sell custom wheels.  They sometimes have the original wheels hanging around that they will sell.

Here is the web address of the complete list.

Start checking in the 'P' range for wheels.
