I'm about to look at this '02 Sierra with 141k miles for potential purchase and the guy is having to wait on its transfer case to be replaced before showing it. I've seen a lot of trucks advertised, but not too many mentioning T case work being done at this early mileage.

Does T case replacement at 141k miles indicate the truck's been overworked with pulling heavy trailer loads? What signs of overuse should I look for on this upcoming inspection, keeping this T case replacement in mind?

Lack of fluid changes could have been a cause for failure, same thing with the pump rub issue that wears through the case and it loses all it's fluid thus leading to failure. The chain can wear out too, it's a mass produced part and things happen.

I wouldn't even think much about it to be honest. Look for other wear on the truck. Check the body and frame for rust, check the brake lines to rust.

I agree with Cam, leaking transfer cases are common resulting in premature wear. Brake lines on a truck of that age would be a common problem regardless of the area , not a game changer IMO

My 02 avalanche was garage kept and looked new when I got it. It still does. It had 167K miles. I did all fluids right away. In the 3 years I kept it with me. I had to replace sticking calipers, the third stop light and brake light switch. Just before I got it a water pump and maintenance items like plugs. Also the oil pressure sending unit. It stayed in the Houston area.

Gotta love AI. Apparently the whole front end flipped upside down, but they were somehow still able to remove the transmission from the front. And that's not a dirt floor like most GM dealers around here.

These older trucks had magnesium cases and had what was called a "pump rub" failure.  The gear pump locating tabs on these over time can wear through the rear of the case and cause a leak.  The factory installed a steel clip on the back of the gear pump to prevent wear but that clip often fails and starts pump rub.  Some cases may have been retrofitted with an aluminum rear half as well via rebuilders or aftermarket.

So that would be entirely a possibility as to why it got a transfer case at 141k miles.  Heck, pump rub was seen at even lower mileage than that back when these trucks were 5-10 years old, not 24 years old.

Unless that thing never saw a winter, I'd be more worried about the condition of the frame, brake lines and fuel lines over the fact that it got a transfer case.
