The cuts in the housing/cup need to be 90 degrees from where they are now. You need to visualize how during suspension travel the balljoint will need to move left or right (when viewed from the front or back). Those cuts in the cup of the balljoint allow for full travel/movement of the balljoint. With the threaded portion striking the raised area of the cup, that was quite possibly the new limitation in suspension travel and after enough stress, it gave out.

Yep. It should be turned 90* or 1/4 of a turn from where it is now. Should be marked, showing the inboard side of the ball joint. Some BJs even have the words "MOUNT INBOARD" stamped on them and on the rubber boot so the relief slot is away from the rotor
